Narrative for Encompass Health Rehabilitation Hospital of Braintree

Braintree Rehabilitation Hospital is an acute rehabilitation facility that focuses on the neurologically impaired patient. As such, our major programs are Stroke, Brain Injury and Movement disorders and at any given time 75% of our patient population is cognitively impaired with balance and gait issues.

We consistently monitor our performance indicators both internally as well as externally through Patient First, Massachusetts Hospital Association quality initiative; Universal Data System Functional Index Measure as well as internal monitoring of quality measures that we have identified as important to Patient Safety.

The Patient First indicators for acute rehabilitation hospitals are Fall Rate, Fall Rate with Injury and Facility-acquired Pressure Ulcers. These 3 indicators provide insight into the care provided by nursing staff. Braintree rate of falls with injury and pressure ulcers prevalence is significantly less than our peer group. We feel that our success is directly related to our commitment to provide the least restrictive environment possible promoting mobility and independence. Braintree Rehabilitation Hospital conducts a 100% review of all restraints utilized within the facility. We feel that this close scrutiny has enabled our facility to provide a safe environment that is restraint appropriate. Our safety training with the staff has increased their knowledge in the management of the neurologically impaired patients so that all appropriate interventions and review of alternatives occur prior to the utilization of a restraint.
We encourage those seeking acute rehabilitation to consider the following: The BRH fall rate is 5.11 and peer rate is 5.18. The BRH fall rate with injury is 0.12, with the peer rate 0.74. The BRH pressure ulcer prevalence rate is 1.0, with the peer rate 2.35. The BRH restraint rate is 0.0007%.

It is our ability to provide quality programs that challenge patients to achieve the highest level of mobility and independence in a restraint appropriate environment which is the fourth indicator that completes the picture.
